Sliding Fee Discount Program (SFDP)

Prevail Behavioral Health & Wellness Center is committed to providing high-quality behavioral health care to everyone in our community, regardless of their insurance status or ability to pay.  There is a discounted/sliding fee schedule available based on family size and income.

No Denial of Care: No one will be denied access to services due to inability to pay.

Non-Discrimination: Discounts are applied regardless of age, gender, race, sexual orientation, religion, disability, or national origin.

Broad Coverage: The discount applies to all primary and preventive clinical services provided at our locations.
